The aim of this website is to provide you with information on the progress of the HealthPathways Sydney Program. The Program began as a collaboration between Sydney Local Health District and Inner West Sydney Medicare Local. Following the closure of Medicare Locals, HealthPathways Sydney has established a partnership with the Central and Eastern Sydney PHN which supports clinicians in our local area.

This site includes planning documents, details on key events, progress on pathway and local resource development and ways in which you can contribute to the program or propose future pathways. 

You will also find information on the Sydney Local Health District's GP e-Referral Program and Project ECHO which alongside HealthPathways are helping to align primary and secondary care models in the Central Sydney region.

 

 

What is HealthPathways?

HealthPathways is a web-based information portal supporting primary care clinicians to plan patient care through our primary, community and secondary health care systems. The HealthPathways site is designed to be used at the point of care, primarily by General Practitioners but is also available to Hospital Specialists, Nurses, Allied Health and other Health Professionals within Inner West Sydney. How can I access HealthPathways?

It's as simple as clicking on the Access button on this page. THis will take you to the HealthPathways Sydney website where you will find over 900 locally relevant Clinical pathways, medication resources and referral information pages. The website is encrypted so you will have to apply for a username and password, this is simple and the landing page will guide you through a request for access. Otherwise if your colleagues are already using healthPathways ask them for the access details.
