SLHD GP e-Referral Project

Sydney Local Health District (SLHD) is delighted to partner with HealthLink to enable electronic referrals directly from your GP medical records systems.

Using the HealthLink SmartForm platform GPs can efficiently send secure e-referrals to a wide and growing range of SLHD Ambulatory Care Services and clinics at Balmain, Canterbury, Concord and Royal Prince Alfred Hospitals. And across 2026 more SLHD services will be accessible via HealthLink SmartForms embedded in Best Practice, Genie and Medical Director software or via the HealthLink Portal. Where to find referral forms on the HealthLink Referral Services & Directory webpage

Currently Sydney Local Health District eReferral forms can be found in two folders on the the HealthLink Service listings page:

  • Sydney LHD Hospitals Services
  • Sydney LHD Aged Care, Allied Health and Community Services

Sydney LHD Hospitals Services

Referrals submitted via HealthLink to these services can be updated from your practice software whilst the referral is still active. With a patient record open go to the Update (Referrals) Tab, on the HealthLink services page to locate the eReferral requiring updating or to provide additional pathology and radiology reports. SLHD eReferral Roadmap

Across 2026 and 2027 additional Sydney Local Health District services will be connected to the HealthLink SmartForm system for GP, AHP and Specialist eRefrrals. Alongside this roll out existing HealthLink connected services that currently connect to both the RMS Lite and Engage Outpatients referral management platforms will be migrating to a new referral management platform within the NSW Health Single Digital Patient Record (SDPR) record system.
