SLHD GP e-Referral Project

Sydney Local Health District (SLHD) is delighted to partner with HealthLink to enable electronic referrals directly from your GP medical records systems.

Using the HealthLink SmartForm platform GPs can efficiently send secure e-referrals to a wide and growing range of SLHD Ambulatory Care Services and clinics at Canterbury, Concord and Royal Prince Alfred Hospitals. And across 2023/4 more SLHD services will be accessible via HealthLink SmartForms embedded in Best Practice, Genie and Medical Director software or via the HealthLink Portal. Across 2024 - 2025 additional Sydney Local Health District services will be connected to the HealthLink SmartForm system for GP eRefrrals. Alongside this roll out existing HealthLink connected services will be migrating to the NSW Health Engage Outpatients referral management platorm. During this period there will be minor changes made to the HealthLink referral form templates to align with the Engage Outpatients platform. General Practice will continue to use HealthLink SmartForms  for referral to SLHD services.

If you do not have any of these compatible Practice Softwares, or you do not have HealthLink, please contact the HealthLink registrations team by calling 1300 14 54 65 or by email to register@healthlink.net and they will be able to assist with setting up an account free of charge and where necessary, access to the referal via the MyHealthLink Portal.
